Waiver and Release of Liability Form: I hereby declare that I understand and assume all risks involved in participating in the submission grappling tournament called ARENA, including but not limited to, the risk of serious injury, paralysis, or death. I further acknowledge that I will be competing at the host location, Droogs MMA and Jiu Jitsu, at my own risk. I understand that the sport of submission grappling and related activities may cause bruising, sprains, fractures, or other injuries to me. At the same time, I also declare myself to physical capable of performing in the tournament and free from any disease that may prohibited me from strenuous activities.I fully assume these risks and waive any and all claims I may have against Droogs MMA and Jiu Jitsu, the ARENA tournament, and any affiliated parties, including their instructors, officers, agents, servants, contractors, or volunteers, for any injury or damages that may arise during or after my participation in this event or class. I also agree to follow all the rules and regulations set forth by the tournament. Video & Photographic Release: I hereby consent to the photographing, audio recording, or filming of my participation in ARENA by Droogs MMA and Jiu Jitsu or third-party media. I understand that these images and audiovisual recordings may be used for promotional, educational, or commercial purposes without any compensation to me. I hereby waive all rights to inspect or approve the finished product and any matter included therein nor solicited. I have carefully read the foregoing release and understand its contents. I agree to this release and sign it voluntarily on the date signed below.

I understand once submitting this application I agree to compete if selected by the Arena team and that the drop out policy is therefore in effect as well as late arrival policies. I understand the event is hosted in Pottstown, PA and have adaquit transportation to attend. I understand I may only bring one pre-registered coach and no spectators of any kind without express permission given.
